How to register as a Citident NHS patient
Thank you for your interest in joining Citident Dental Practice.
We try to make the process as seamless as possible.
- Please note your first NHS appointment is only a checkup!. If you are in pain or would like an urgent appointment we can see you as a private patient, please call us on 01908 606 666.
- Our Receptionists will upload your details to our Dental Software. This can take up to 72 hours.
- After 72 hours you, call us on 01908606666 and one of our friendly team will be able to book your first check up appointment!
Please bear in mind we will only store your information for 2 weeks. If you have not booked a check up appointment by this time, your information will be destroyed.
Please be aware we have a database of our regular patients who come in for regular checkups and treatment. Filling in this form does not mean we are your dentist or can offer you emergency appointments on the NHS until you come in for your first checkup.
Finally, if you have any exemption from regular NHS charges please bring proof of your exemption to your first appointment to avoid any penalty charges from the NHS.
